This form helps the specialist to “scrape out” subcutaneous fat in the armpit, thereby destroying sweat glands: they are the cause of hyperhidrosis. The operation is performed under local anesthesia. The surgeon determines the boundary of the area of ​​excessive sweating and makes a tiny five-millimeter incision through which fat with sweat glands is removed. This is the slight drawback of curettage: it leads to a loss of sensitivity in the axillary region. Disadvantages of this procedure include minor bleeding, hematomas, and the potential for re-curettage after a few months. In general, surgery is effective in 90% of cases, but its main drawback is the need for surgical intervention.

Sympathectomy

Sympathectomy is used mainly with increased sweating of the palms or combined hyperhidrosis (for example, the entire upper half of the body). But this procedure is less common in our time. The reason for this is the difficulties that the doctor encounters during surgery. Therefore, endoscopic sympathectomy is much more often used now, when the size of the incision is limited to only one centimeter, sufficient for the introduction of an endoscope. During the procedure, the sympathetic trunk is stopped (nerve endings located on both sides of the spine), which thus ceases to send nerve impulses to the sweat glands, and the areas above the nerve intersection become dry. In a small percentage of cases, there is such a complication as compensatory hyperhidrosis – when, after reducing sweating in the upper half of the body, sweating in zones below the waist increases significantly. Contraindications for sympathectomy are diabetes mellitus and other endocrine and infectious diseases. In general, with hyperhidrosis, the best option is to immediately engage in the treatment of the disease that caused it. When you get rid of the disease, excessive sweating can also disappear.

Botulinum toxin

Botulinum toxin injections are the most universal but at the same time more expensive than curettage and sympathectomy due to the need for regular repetition of the procedure. They can be used not only for auxiliary and palmar hyperhidrosis but also throughout the body. The drug blocks the transmission of impulses to the sweat glands. A big advantage is the absence of surgical intervention. The effect of the treatment with botulinum toxin will last an average of six months, after which the procedure will need to be repeated. Contraindications to injections include hypersensitivity to the components of the drug, myasthenia gravis (a progressive neuromuscular disease) and inflammation in the injection area. There is a small percentage of people who are not sensitive to botulinum toxin, in such patients the procedure will not be effective and they should pay attention to other methods of treating hyperhidrosis.
